On or before the day of admission, the resident or responsible party will need to sign the necessary documents to complete the admission process. Please ask to speak to the Admission Coordinator or a member of the social service staff for more details.
Meeting Each resident is evaluated by our interdisciplinary team. Residents and families are encouraged to be involved in developing a plan of care that focuses on achieving agreed upon goals during the stay at Autumn Hills.
We encourage families to bring small personal items with the resident. However, residents should not keep large sums of money or valuables in their room. We are not responsible for the loss or damage of any personal property. A recommended list of clothing is available.
Autumn Hills Care Center is a smoke-free facility. Residents are not permitted to smoke anywhere on campus.
Visitor Parking is located on the northeast side of the building only. Please do not park in the employee parking lot. Visitors may enter through the double doors on the northeast side of the building or the front door. No other entrance is a visitor entrance.

There are several types of payment options for nursing home care including: Medicare (Parts A and B), Medicaid, Private Long Term Care Insurance, Private Insurance Skilled Nursing coverage and Managed Care policies. The payment options are unique to each admission. We will make every effort to ascertain any outside payer source and will make all attempts to assist you in obtaining payment through third party resources. Of course, a resident may also pay from private funds for nursing home care.
We will gladly extend admission privileges to your physician as long as he/she agrees to comply with the many state and federal laws regarding physician care of the nursing home resident. Most physicians choose to turn the care of their residents to the "house" medical staff. Your physician may assist you in this choice, however, the final choice is based on your preference.
