Plan of Care Meeting

Each resident admitted to Autumn Hills Care Center is evaluated by our interdisciplinary team. A plan of care is then developed identifying the resident's current status, goals, and the means to obtain these goals. The initial plan of care meeting is held within ten days of admission, and quarterly thereafter.

Residents and families are encouraged to be involved in the plan of care. A letter is also sent to the family with notification of meeting date and time.
