Autumn Hills Care Center is located in Niles, Ohio, just off of Route 422, between Warren and Youngstown. We are located 1.6 miles east of Eastwood Mall.

From Interstate 80 (north of Youngstown) take State Route 11 North to the Tibbets Wick exit.
If coming from the south, turn left onto Tibbits Wick Road (west). If coming from the north, turn right onto Tibbits Wick Road (west).
Travel 1.5 miles on Tibbits Wick to Route 422. Turn right onto Rt. 422 (west).
Travel 1.2 miles on Route 422 to Niles Vienna Road.
Turn right onto Niles Vienna Road. We are the second building on the left, behind Niles Park Plaza.
