  1. Q: Who Pays For Nursing Home Care? What Insurance Do You Accept?

    A: There are several types of payment available for nursing home care. They include, but are not limited to: Medicare (Parts A and B), Medicaid, Private Long Term Care Insurance, Private Insurance Skilled Nursing coverage and several Managed Care policies. The payment options are unique to each admission. We will make every effort to ascertain any outside payer source and will make all attempts to assist you in obtaining payment through third party resources. Of course, a resident may also pay from private funds for nursing home care.

  2. Q: Can Our Own Doctor Come to Autumn Hills Care Center?

    A: We will gladly extend admission privileges to your physician as long as he/she agrees to comply with the many state and federal laws regarding physician care of the nursing home resident. Most physicians choose to turn the care of their residents to the "house" medical staff. Your physician may assist you in this choice, however, the final choice is based on the resident/family preference.

  3. Q: What Should We Bring From Home?

    A: Prior to admission a list of selected items will be given to the resident or responsible party.

  4. Q: What is the Staffing Ratio at Autumn Hills Care Center?

    A: The staffing ratio varies from wing to wing and shift to shift. However, Autumn Hills Care Center prides itself in the fact that we are always staffed above the minimum state staffing requirements.

  5. Q: Are Telephones and TVs Provided?

    A: No. However, we do allow phones to be installed at the expense of the resident. Residents must supply their own TVs as well, although cable access is provided free of charge to each resident.

  6. Q: How Are Medications Provided?

    A: Residents may select their own pharmacy. However, any pharmacy selected must comply with all state and federal regulations as they relate to long term care facilities. These regulations will be explained in depth upon admission.

  7. Q: What Are the Different Levels of Care Provided at Autumn Hills Care Center?

    A: 

    We are proud to offer a variety of care levels in our facility. Some common terms you may hear are:

    Subacute Care - This care level is for residents who are admitted directly from an acute care setting (hospital) and have multiple medical problems. The care level is very high, requiring a myriad of complex medical services. Medicare Part A may pay for this care level as well as private insurance or managed care insurance.

    Skilled Care - The skilled care resident requires 24 hour nursing care. Generally this care is of a more technical nature than the family can handle at home. This care level may qualify a resident for limited Medicare coverage. The determination of the resident's stay under Medicare is made by the facilities Utilization Review committee. Private insurance and managed care coverage may also pay for this type of care.

    Intermediate Care - The majority of our residents require intermediate care. Although they may be able to function with some independence , these residents may require assistance from the professional staff in bathing, mobility, eating or activities. Medicaid, private payments, or long term care insurance are the resources available for this care level.
