Discharge Planning

Discharge planning begins upon admission. Our discharge team consists of the physician, therapists, nurses and social workers. They will assist you in making a smooth transition from our facility to the community.

Services provided include resident-family teaching according to the individual's needs and arranging for home-health services and equipment.
